Upbeat Nystagmus During Head Rotation in Rotational Vertebral Artery Occlusion

Abstract
Rotational vertebral artery occlusion (RVAO) is characterized by recurrent attacks of paroxysmal vertigo, nystagmus, tinnitus, and syncope induced by horizontal head rotation. The induced nystagmus in RVAO was mostly mixed downbeat and horizontal with or without a torsional component. We report upbeat nystagmus induced by head rotation in a patient with RVAO. The upbeat nystagmus in our patient can be explained by asymmetrical inhibition of bilateral anterior semicircular canals due to ischemia or by transient excitation of the inferior cerebellum due to hypoperfusion. Alternatively, transient ischemia of the brainstem may be invoked to explain the vertigo and nystagmus.
